Community Medic Program
The Satellite Beach Fire Department offers innovative services designed to assist City residents to stay in their homes independently, longer, safer, healthier, and happier. The healthcare system is a complicated process. These patients generally have multiple physicians, get their medications filled at different pharmacies, and usually do not have family or an advocate nearby to assist them.
The Satellite Beach Fire Department offers FREE in-home health & home safety evaluations for older citizens who have chronic health conditions but may not require the services of the Community Medic Program (CMP) on an ongoing basis just yet. If requested, the medics will schedule an appointment to visit residents in their home and conduct a FREE wellness and home safety check up with the goal of providing information about the Community Medic Program.
The program is intended to bridge the gap between regular visits with your Primary Care Physician, and to identify and mitigate situations that may otherwise lead to a medical emergency.
Program Goals (Aim):
• Enhance the overall patient health care experience, including quality outcomes and customer satisfaction
• Improve the overall health of our population
• Reduce overall healthcare cost
Services Provided:
• In-home medical service appointments routinely scheduled
• 12 Lead ECG, when medically necessary
• Vitals including weight, pulse, blood pressure and oxygen saturation statistics
• Blood glucose testing, when medically necessary
• Medication review and compliance review
• Nutrition review
• Link with Primary Care Provider(s) to provide first-hand observations about your health status and identify needs
• Link to Community Services to enhance your quality of life
